Position Summary: With a focus on renal transplant and hepatobiliary patients, this Ambulatory RN II is a member of a multidisciplinary team that provides advanced professional nursing care to those in varying states of health and illness through assessment, planning, implementation, and evaluation of the patient's health needs. Daily responsibilities involve direct communication with patients and providers related to clinic appointments, ongoing surveillance of health status through labs and procedure results, and coordinating follow up as needed. This nurse coordinator provides education to potential transplant recipients and living donors. S/he will attend hospital rounds and work closely with newly transplanted patients on all aspects of preparing for a safe discharge. This position works closely with surgeons and nephrologists as well as specialty pharmacists, dieticians and social workers.
The Ambulatory RN II provides direct care to patients and families, as well as teaching and preparation for continuing care. Organizational Standards of Patient Care and Nursing Practice guide the delivery of nursing services by this individual. The position demonstrates dedication to professional growth and development and to the continuous improvement of patient care.
This position involves significant time spent on the computer and phone. The successful candidate will also take call approximately one week per month (a shared responsibility with three other coordinators).
